The pupose of this form is to make permanent group or meeting changes. This includes:

• Meeting location change
• Meeting day and or time change
• Meeting format/type change, such as closed discussion, open discussion, literature, etc
• Meeting name change
• GSR, Alternate GSR, or SMIA Rep change
